I have never had reflect or teleport items drop (I have found some invisibility items), in spite of the fact that I have killed hundreds (thousands?) of ancient wyrms, balrons, dragons, deamons, elementals, etc). I heard from somebody, however, that they are not uncommon in dungeon chests, but you need a lockpicker for that. I have seen a few from treasure maps, but in the 10-20 that I've done on this server, I've only seen 1 or 2. So it sounds like dungeon chests are the way to go for those.
