So I was killed twice now... in this exact spot - while macroing healing. I cannot seem to figure out how; no pets or npcs were present.
Any ideas would be greatly appreciated.

Assuming you were injuring yourself in some way to practice healing....
AFAIK the earthquake spell can reach you there but not kill you, however if someone took your health down low enough with earthquakes and then you injured yourself, you might have killed yourself with your own macro?
