real vs "unreal" skill level

which one is the one that actually matters? i.e. - you need 60 minimum skill to have 80% chance of curing poison. 60 real or 60 "unreal"?

Shown skill matters in basically all cases. The shown skill is your base skill plus any stat bonuses you get. As your skill increases those bonuses decrease and eventually disappear.

If you have, say, 58 real and 61 adjusted healing you would be able to cure poison using healing.

But, if you dropped your stats and went back down to 58 you would not be able to cure poison.
